This Terms of Service Agreement (the "Agreement") is entered into by and between The Bridge Health Advocates (hereinafter referred to as "Association") and you, the user (hereinafter referred to as the "User"). This Agreement sets forth the terms and conditions governing the use of the Association's services and website.

Program Updates and Appointment Reminders By providing your phone number, you agree to receive occasional program updates and appointment reminders by SMS text message. These SMS text messages may be sent using an automated system. Standard message and data rates may apply. You can opt out at any time by typing STOP in response to the received text message.

Use of Services The User agrees to use the Association's services and website in compliance with all applicable laws, regulations, and this Agreement. The User acknowledges that any unauthorized use or access to the Association's services or website may result in termination of access and may give rise to legal action.

Intellectual Property All content, trademarks, logos, and intellectual property rights associated with the Association's services and website shall remain the exclusive property of the Association. The User agrees not to reproduce, distribute, modify, or create derivative works of any materials or content without prior written consent from the Association.

Limitation of Liability The Association shall not be liable for any indirect, consequential, incidental, or punitive damages arising out of or relating to the use of its services or website. The User agrees to use the services and website at their own risk.

Indemnification The User agrees to indemnify and hold the Association and its affiliates, directors, employees, and agents harmless from and against any claims, liabilities, damages, losses, or expenses, including reasonable attorney fees, arising out of or in connection with the User's use of the Association's services or website.

Amendments to the Agreement The Association reserves the right to modify or revise this Agreement at any time. The User's continued use of the services and website after any such changes shall constitute their acceptance of the revised Agreement.

Governing Law and Jurisdiction This Agreement shall be governed by and construed in accordance with the laws of the state of Arizona. Any disputes arising out of or relating to this Agreement shall be subject to the exclusive jurisdiction of the courts located in the state of Arizona.
